Diane has been a RN for over 25 years. She currently lives in Gettysburg PA and is a Senior Clinical Consultant for home health and hospice with BlackTree Healthcare Consulting. Diane has a Master’s Degree in Healthcare Administration from Mount Saint Mary’s University in Maryland and maintains her RN license in Maryland, Pennsylvania and New Jersey. She has a Green Belt in Lean Six Sigma.

Diane is president of the board for the Maryland National Capital Home Care Association. She has received awards from the MNCHA over the years for her leadership and health care policy advocacy. Diane has published the Emergency Preparedness Manual for Home Care Providers in Maryland.

Lower extremity amputations attributed to complexities of the diabetic foot are well demonstrated to be preventable. Knowledge regarding current preventive strategies is not widely understood or taught to staff nurses.  This is part 1 of a 2-part series on Diabetic Foot Care.

Education Overview
This presentation focuses on identifying the foot at risk for foot ulcers and lower extremity amputation, and provides an organized prevention program model for agencies and facilities to incorporate in their Diabetes Education Programs. Areas of key focus are (1) foot screening for Neuropathy and PAD, (2) Stratifying patients into management categories based on their level of risk and (3) Managing patients with routine provider assessment and daily patient self-care. The essential role of Patient Self-Management Foot Education and Care, as well as the role of appropriately designed and sized footwear is also discussed. Learning objectives for this session are:

  • Discuss 3 key components of a comprehensive foot exam for the patient with diabetes.
  • Identify two commonly seen risk factors among patients at risk for lower extremity amputation

Myra Varnado, BS, RN, CWON, CFCN, Director of Clinical Wound Services for Corstrata, is a Wound, Ostomy and Continence Nurse Specialist with extensive clinical expertise in Pressure Ulcer and Diabetes Foot Ulcer Prevention and Management. She graduated from the Emory University WOC Nursing Education Program and has authored numerous publications on WOC topics. On behalf of the Wound, Ostomy and Continence Nurses Society, she authored the Clinical Practice Guidelines that appear on the AHRQ website. For the past 16 years, she has served on the WOCN Wound Guidelines Task Force and is the primary author of the WOCN Clinical Practice Guideline: “Management of Wounds in Patients with Lower Extremity Neuropathic Disease”.

The Medicare Home Health Cost Report (MCR) is a form similar to the Tax Return in that it was designed to enable the demographic, operational & financial information for the thousands of Home Health Agencies (HHAs) across the nation to be presented in one standardized format for analysis and review. 

The MCR has long been undervalued and underestimated as to its impact on regulatory and reimbursement changes.  This predates the Prospective Payment System (PPS).  But since the inception of the OASIS document, and especially since the inception of PPS, the MCR has been a tool that has greatly shaped (both positively and negatively) our regulatory and reimbursement changes (e.g., the MCR is used to identify Medicare Costs and Profits, and they were used as the foundation for the Rebasing of HH PPS and the HHGM proposal).
