Start the New Year with this 90-minute session designed to help agencies improve their OASIS-Based HHVBP components using real life examples.  Are your clinicians providing great care, but your anticipated scores are not what you hoped for?  Sara will break down each item and give practical examples of how to maximize improvement scores wherever possible with focused education and simple audits.  This webinar uses a back to basics, train the trainer, clinically focused format offering strategies to position your agency for success in the new world of Value Based Purchasing.  Registered participants will receive a Clinician Tip Sheet as well as a Targeted Clinical Audit Tool. 

Program Objectives:

• Understand the OASIS item detail within the HHVBP TNC Self Care and TNC Mobility Measures
• Understand the Improvement In Dyspnea and Improvement In Oral Medications items in intent and detail for VBP and Star Ratings
• Understand HHVBP improvement score calculations as compared to benchmarks for TNC Self Care and Mobility Measures
• Be able to implement important clinical assessment techniques to score patients appropriately on SOC/ROC and Discharge to capture real improvement in functional status.
• Walk through a Targeted Audit to examine where to focus improvement efforts for your agency
• Receive a Clinician Tip Sheet to hand out to staff
